Swing Bed: Call to Action - Facility Assessment and Competency-Based Approaches

CMS and nursing homes/swing bed programs are initiating and integrating the most changes to the core components of skilled nursing care, documentation, and reimbursement in 32 years. With new quality measures, updates to the Five-Star rating, and regulatory changes adding to MDS reporting time and accuracy requirements, this is a call to action considering the impact of these significant changes to PPS/rural hospital swing bed programs (and all nursing homes). The burden will fall equally on both PPS and CAH swing bed programs, each setting will need to address their process for holding their Swing Bed program accountable.

Education and training will address new swing bed program considerations and address challenges with significant staff turnover. Additionally, there will be sessions focused on issues impacting survey, reimbursement, and patient satisfaction. The Rural Swing Bed Management Program offers a combination of webinars, technical assistance check-ins, and in-person interactive workshops to discuss building and maintaining a compliant swing bed program with measurable financial expectations and quality outcomes. The program is designed to provide interaction with other swing bed program best practices, while also providing individual hospital assistance.

Led by industry expert, Kerry Dunning, alongside Hometown Health’s educational support team; the Rural Swing Bed Management Program include the following components which will be available in the HomeTown Health Track 2 consortium:
• Biannual Live Training in April and October
• Access to HTHU School of Swing Bed Management Certificate Program which support

CAH and PPS swing bed programs’ understanding of the intent of the swing bed level of care and the skilled nursing regulations needed to operate and financially manage the program. This Certificate Program will help develop participants' understanding of swing bed foundations and provide proven best practice solutions for program implementation and success.
