In this certificate program, students learn how community partnerships that deploy community-based care givers such as paramedics and community health workers, can positively impact the overall health and chronic disease management of individual citizens.

It is known that drivers of individual and population health are social, economic, environmental, and behavioral. As these factors are largely determined by where people live, work, and play—communities nationwide are testing new partner strategies and interventions to address the social determinants of health and disease management.

The focus of this certificate program is to examine the role of community health workers and paramedics serving as front-line public health workers and to discuss how telehealth technologies could be leveraged to improve access to care.
