IOWA ON-SITE VISITS 2019-2020

It is the goal of HomeTown Health and other Consortium partners  to visit each participating hospital by the end of the six year period, or about 12-13 hospitals per year.   These grant onsite visits, separate from project onsite visits, will likely consist of  a half-day visit from HomeTown Health, IDPH, and two consortium trainers/consultants. The purpose of the informal visits it to get to know your hospital team, your needs, ensure you are taking advantage of all the resources available to you.    In addition, we hope to include at least one if not two subject matter experts to provide some time with your team on the financial or quality side.

Hospitals were asked to volunteer for year one onsite visits.  We received numerous invitations to participate in year one.  Based upon several other factors of involvement, HomeTown Health has selected the first group of hospitals we would like to visit in the 2019-2020 grant period.  The additional list of hospitals are included to be first on the list for the 2020-2021 grant period, and will begin as soon as June, 2020.
