HomeTown Health is pleased to serve as the manager of the Iowa SHIP Grant Program in partnership with the Iowa Department of Health & Human Services. Hospitals who have attested and opted for Direct Award through the Iowa Department of Health & Human Services for Year 2026-2027 have the following responsibilities throughout this year:

• Submit Direct Award Agreement by July 24, 2026.
• Submit Mid-Year and Final Reports

Please Note:  If your facility would like to change out of 'direct award' to Consortium Participation, you must declare that change by July 24, 2026. Any facility that does not complete their letter of agreement for direct funds by July 24 will be moved into the Consortium.
