It is important to understand the true purpose and importance of medication reconciliation in order to build an effective process. This portion of the course will dive into the key components of the medication reconciliation process during the hospital admission process and patient ER episodes of care. It will assist attendees in understanding how to collect and verify patient information to create a thorough admission medication list that will follow the patient throughout the entire patient stay. It will also help attendees recognize how to include healthcare clinic and/or care coordination staff in the med rec process, and the roles that each of these participants play. Finally, we will conclude the presentation with a focus on the role that pharmacy plays in the entire medication reconciliation process from the pharmacist's perspective.

Attendees will recognize how to assess the needs of the patient surrounding medication, and how to build an effective education plan that addresses all the patients needs while accommodating for patient preferences and barriers to learning. Also, it will assist attendees in identifying resources and tools that can assist them in the education process. Medication reconciliation does not just occur inside a healthcare setting, as there are many aspects to medication safety that exist for a patient within their home. This portion of the course will provide a look into processes that can be implemented to patients that are either at home or for patients that have discharged from an inpatient setting back to home. It will also help to see a clear picture of how to combine all of the key elements of the med rec process from across the continuum of care, into one collaborative process. One of the key components of a successful patient discharge is to have an effective medication reconciliation process in place. Healthcare settings must understand the true importance of Medication Reconciliation and how and where to implement it within the discharge process. This course will teach attendees how to effectively educate and prepare their patients for a successful discharge. Attendees will learn what key elements need to be included when creating a thorough discharge medication plan, as well as key services and resources to arrange to assist the patient during and following discharge. The course will also address the importance of Pharmacy and the role it plays for medication reconciliation within the discharge process.
